What actually happens in a first session

By Dr. Cynthia Manguba, PsyD·July 14, 2026·4 min read

One of the most common reasons people wait years to start therapy is simple: they don't know what they're walking into. So let me take the mystery out of it.

A first session is not a test. You will not be asked to relive your worst day or explain yourself perfectly. Mostly, it's a conversation. I want to understand what brought you here, what you're hoping might feel different, and a little about the life you're living right now.

You are allowed to not have the words yet. You are allowed to say, honestly, that you're not sure why you came, only that something feels off. That's enough to begin with. Finding the words together is part of the work.

By the end, we'll talk about whether we feel like a good fit, and what a rhythm of working together might look like. There is no pressure to commit to anything in that first hour. It is simply a beginning.

If reaching out feels like the hardest part, that's because it often is. The rest is gentler than you think.
